Official Contest Rules
NO PURCHASE NECESSARY TO ENTER OR WIN. A PURCHASE WILL NOT INCREASE YOUR CHANCE OF WINNING. VOID WHERE PROHIBITED.
1. How to enter:
a.) The Shamrock Family Vacation Sweepstakes Contest ("the Contest") will begin on March 1, 2008 at approximately 12:00 a.m. CT and end on March 31, 2008 at approximately 11:59 p.m. CT ("Contest Dates")
b.) To enter: You'll automatically be entered in The Shamrock Family Vacation Sweepstakes every time you purchase a participating vendor product when you use your Jewel Preferred Card at any participating Jewel Store in Illinois, Indiana, and Iowa between the dates of March 1 and March 31, 2008.
c.) Alternate Methods of Entry via Mail: No Purchase Necessary - Void where prohibited.
a. During the contest dates, eligible consumers may enter on a plain piece of 3" x 5" paper, hand print your full name, complete address including zip code, daytime phone number, email, and date of birth in order to be eligible for a chance to win.
b. Eligible consumers may also fill out an official contest entry form found at participating Illinois, Indiana, & Iowa Jewel Osco locations in the Spotlight ad while supplies last.
c. Eligible consumers may also complete an entry form at www.mercyhome.org and submit entry via mail.
d.) Mail completed entry in a First Class stamped envelope to: Shamrock for Kids Family Vacation Contest; 1140 W. Jackson. Chicago, IL 60607.
a. Entries must include full name, complete address, including zip code, daytime telephone number, e-mail and date of birth to be eligible for a chance to win.
b. One (1) entry per person. No photocopies or mechanically reproduced entries will be accepted. One entry per outer mailing envelope.
c. Mail-in Entries must be postmarked by March 31, 2008.
e.) Mercy Home for Boys and Girls, and the promotional partners of this Contest (hereinafter collectively referred to as the "Sponsors") assume no responsibility whatsoever for any lost, misdirected, illegible, stolen, incomplete, mutilated, or late entries.
f.) All entries become the exclusive property of Mercy Home for Boys and Girls and Jewel Osco and will not be returned.
2. Eligibility Restrictions:
a.) The contest is open to all consumers who are 21 years of age and older at the time of entry, and who reside in the Chicagoland, Northwest Indiana and Iowa area. Employees of Mercy Home for Boys and Girls, Jewel Osco, and individual participating Jewel Osco locations, and its corporate parent, and affiliates, their advertising agencies, participating sponsors/promotional partners, other radio and TV stations in the Illinois, Indiana, and Iowa areas and the members of their immediate families are ineligible to participate or win.
b.) Entry into the Contest constitutes your agreement to be bound by these Official Contest Rules.
c.) Winner may be required to sign a participant release in order to claim prize.
d.) Winner must provide valid government-issued photo identification to be eligible for receipt of prize.
3. Prizes:
a.) One (1) Grand Prize Winner will receive four (4) round-trip main cabin ("T" Class) travel award letters to Ireland aboard American Airlines: Chicago O'Hare to Dublin based on space availability good for one year from April 1, 2008 - April 1, 2009. Four (4) night hotel accommodations at designated property in Ireland will be awarded with airtransportation, based on space availability, good for one year from April 1, 2008 - April 1, 2009. Approximate retail value of Grand Prize ("ARV"): (1) Grand Prize - $8900.00, ARV: Cost
b.) Travel Award letters (s) have no value except when redeemed in accordance with the terms and conditions in the prizeletter(s) guidelines as well as those listed on the back of the prizeletter(s). Tickets are not transferable or refundable.
c.) Reservations made using prizeletter(s) are subject to availability of promotional inventories. Coach must be booked in "T" class, Business in "U" class and First in "Z" class. No open tickets are permitted.
d.) Tickets can only be issued by American Airlines Promotion Department (800-727-2142). Tickets cannot be issued at the airport or by travel agencies.
e.) Travel must originate and terminate in the American Airlines or American Eagle city nearest the city of residence providing direct service. Direct service is travel on a published routing between two points, and any connection must be a standard connection for that route. Backtracking is not permitted. (Backtracking occurs when any segment of a trip alters the course so that the route is not in the direction of the destination.) Stopovers, except for the next connecting flight, are not permitted.
f.) Travel is subject to applicable blackout dates, which are subject to change. All travel using prizeletter must be completed by designated expiration date. No extensions or exceptions will be allowed.
g.) Passengers cannot utilize AAdvantage mileage to upgrade or pay for an upgrade from a free ticket.
h.) Transportation on other carriers, ground transportation and overnight lodging, excess or oversized baggage, if required, will not be at the expense of American. Minors must be accompanied on the flight by a parent or guardian.
i.) If American, for any reason, discontinues service to the selected destination before the travel date, the user must select an alternate American Airlines destination.
j.) Any changes, revisions, or credits for prizeletter(s) or unused tickets are subject to additional administrative service fees and must be approved by American Airlines Marketing Promotions Management.
k.) After a ticket has been issued, a service charge of $100. per ticket will be assessed for any changes requiring re-ticketing.
l.) All unused Travel Award letter(s), or tickets must be returned to American Airlines Sales Promotion. Credit will not be given for partial ticket returns. Lost or stolen prize letter (s) will not be replaced.
m.) Any applicable fees, taxes, surcharges, service charges, or passenger facility charges are the responsibility of the user and must be paid at the time the ticket is issued.
n.) Mercy Home for Boys and Girls is responsible for issuing 1099 tax forms to winners based on prize value.
4. Selection of Winners and Notification:
a) Decisions of Mercy Home for Boys and Girls management with regard to this contest are final.
b) One (1) winner will be drawn at random from among all eligible entries received on April 7, 2008, at approximately 5:00 p.m. C.T. at Mercy Home for Boys & Girls on Jackson Blvd. Chicago.
c) Odds of winning depend upon the number of eligible entries received, during the contest dates.
d) Winner need not be present to win.
e) Winner will be contacted via telephone by a Mercy Home for Boys and Girls Communications Department Representative immediately following drawing with further details on claiming the prize.
f) Should the winner be found ineligible to receive the contest prize, he/she will forfeit the prize and Mercy Home for Boys and Girls reserves the right to select an alternate winner by random drawing of all remaining eligible entries.
g) Winner must execute and return any required Affidavit of Eligibility and/or Release of Liability Prize Acceptance Form within three (3) days of notification attempt or prize will be forfeited and an alternate winner may be selected.
5. Conditions:
a) Payments of all federal, state and local taxes are solely the responsibility of the winners. Winners will be required to sign an IRS Form W-9 or the equivalent.
b) By participating in any contest, winner agrees to have their name, voice and/or likeness used in any advertising or broadcasting material relating to the contest without additional financial or other compensation, and where legal, to sign a publicity release confirming such consent prior to acceptance of the prize.
c) Prior to awarding any prize or prize certificate, Mercy Home for Boys in its sole discretion may require contest winner to sign a publicity/liability release agreeing to hold Mercy Home for Boys and Girls, Sponsors, their corporate licensee, parent corporations, subsidiaries and affiliated corporations, and the officers, directors, shareholders, employees, agents, and representatives of each of them harmless against any and all claims or liability arising directly or indirectly from the prize or participation in the contest.
d) By accepting the prize, the prize winner represents that he/she understands that enjoyment of the prize awarded by this contest may contain some inherent elements of risk of accident or injury, which may be caused by accident, negligence or forces of nature. Prize winner represents that he/she recognizes that such risks may be present at any time before, during and after accepting the prize. In order to be considered a Grand Prize winner and partake of the activities of the event, each potential Grand Prize winner must be willing to accept the risk, and to confirm his/her acceptance to such risk in writing at Sponsors' request.
e) Mercy Home for Boys and Girls reserves the right to make changes in the rules of the contest, including substitution of a prize of equal value, which will become effective upon announcement
f) Rules are available for viewing by logging on to http://www.mercyhome.org/, and at the Mercy Home for Boys main campus located at 1140 W. Jackson Chicago, IL 60607, from 9:00am to 5:00pm central time.
g) Failure to comply with the contest rules may result in an entrant/contestant's disqualification solely at the discretion of Mercy Home for Boys and Girls management.
h) If due to circumstances beyond the control of Mercy Home for Boys and Girls reserves the right, but not the obligation, to cancel or modify the contest and shall not be required to award a substitute prize.
i) Mercy Home for Boys and Girls and Illinois, Indiana, and Iowa Jewel Osco locations are not responsible for typographical or other errors in the printing, the offering or administration of the contest or in the announcement of a prize.
j) WINNERS LIST: For the name of the winner, available after April 6, 2008, send a separate, self-addressed, stamped (#10) envelope to: Mercy Home for Boys & Girls, Shamrocks for Kids Family Vacation Sweepstakes, 1140 W. Jackson Blvd., Chicago, IL 60607. All requests must be received by April 30, 2008.
